Coffee bean sign large bowel obstruction. Volvulus, from the latin volvere (to turn), is an abnormal twisting or torsion of a loop of bowel on itself, causing intestinal obstruction that can lead to hemorrhagic infarction, perforation, or death. Surgery is recommended for patients with signs of bowel gangrene. The remainder of the large bowel is not dilated, presumably because the proximal point of the twist is not causing obstruction and thus allows drainage into the sigmoid.
